            Organization Name

            Senior Pastor or President of organization

            Number of Volunteers

            Group Leader

You will be contacted with further information regarding Orientation, Registration, and authorized events.

• Past events from the 2008 season included:

            Mary J. Blige/Jay-Z concert

            Jonas Brothers concert

            Faithful Central Praise Party 2008

            USC Rose Bowl game

• Fundraising program allows participants FREE access to major events while raising funds for organizations

• Program operates in the Los Angeles area and the Orange County area

• Groups of ten individuals are dispatched to events with a designated Group Leader

            Volunteers must be 18 years of age or older with two forms of valid identification

            Volunteers can have no felonies on their record

• Each Group member will receive:

            OMInc. Volunteer file number

            CSC Employee number

            Transportation to event and from event or directions

            Complimentary Lunch*

• OMInc. may compensate Group at an hourly rate per individual as a fundraising payout based on attendees and agreement with Group Leader(s).
